Your First Visit
Your appointment will consist of a consultation explaining your diagnosis and treatment options. Treatment is typically done immediately following the consultation. However, a complex medical history or treatment plan may require that treatment be done at a later date.
Please assist us by providing the following information at the time of your visit:
- Your referral slip
- Any X-rays (if applicable)
- A list of medications you are presently taking
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: A parent or guardian must accompany all patients under 18 years of age.
Please alert the office if you:
- Have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, history of rheumatic fever or heart disease, etc.)
- Are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
- Require medication prior to dental treatment (i.e antibiotic pre-medication prior to cleanings, etc.)
- Have taken prescription medications or injections in the past for bone density
Before Your Appointment
Unless instructed by Dr. Gilliam to do otherwise, please eat a healthy meal, stay hydrated, and take your prescription medications as prescribed prior to your appointment. If there is a question about a particular medication, please call our office prior to your appointment.
If you are currently taking a narcotic or medication that causes impairment, please call our office before coming in person.
